Build your intake form

If your form is long or sounds like a doctor's office, people quit halfway or bend the truth. Then you walk into the call guessing.

How to do it

  1. Use the sections in the checklist below. If a question can wait for the call, cut it from the form.
  2. Write every health question in plain words. Ask "Has a doctor ever told you your blood pressure is high?" not "Do you have hypertension?"
  3. Swap judging words for neutral ones. Ask "What does a normal lunch look like?" not "Do you eat junk food?"
  4. End with a short consent line and a privacy note. Ask your certifying body or insurer what you need to keep and for how long.
  5. Fill it out yourself with a timer running. If it takes more than about ten minutes, cut more.

Example: Maya, an in-person trainer in a commercial gym

Availability: Which days and times can you really train? Circle morning, lunch or evening for each day.

Goal: In your own words, what do you want in the next 3 months? In the next year?

Health: Has a doctor ever told you your blood pressure is high?

Health: Have you ever had chest pain, felt dizzy or fainted during exercise?

Food: What does a normal lunch look like for you?

Privacy: Your answers stay in a password-protected folder. Only I see them.

Timer: 8 minutes. Under ten, so I kept it.

Set up your prep for the day before

A form you skim in the parking lot is a form you didn't need. Prep beats polish: the work you do before the call matters more than how smooth you sound on it.

How to do it

  1. Send the form a day or two before the call as a link or email. Don't pass health forms around by text.
  2. Read every answer the day before the call.
  3. Mark anything that worries you, health answers first.
  4. Write down two questions the form raised.
  5. Cross off any call question the form already answered. If it says they sleep six hours, don't ask how they sleep. Ask what happens on the nights it's worse.

Example: Maya's prep for a sample lead, Dana

Form says: sleeps 6 hours, desk job, knee is "a little off", quit a gym plan twice.

Worry: the knee. I'll ask when it hurts and if anyone has looked at it.

Won't ask: how do you sleep. Will ask: what happens on the nights it's worse?

Question 1: What week did the last two plans fall apart?

Question 2: Who at home knows you're starting this?

Routine: send the form when they book, read it the night before at 8pm.

Write your consultation question sheet

Without set questions you either talk too much or settle for vague answers like "I want to tone up." Let them do most of the talking and save yours for the end.

How to do it

  1. Write the seven questions in the table below on one sheet you keep in front of you on the call.
  2. Next to each one, write the follow-up you'll use when the answer is vague.
  3. Write their answers down in their own words. You'll say them back when you close.
  4. On the 12-week question, keep asking until you can picture it.
  5. Plan on about 30 to 45 minutes: most of it on these questions, the last few minutes on the plan and the price.

Example: Maya's question sheet

Why now? If "I just felt like it": What happened this month that made it feel important?

What would be different in 12 weeks? If "tone up": What would you see in the mirror? What would you wear or do that you don't now?

What have you tried, what made you stop? Then: What week did it usually fall apart?

Walk me through a normal Tuesday, waking up to bed. I check it against the sleep answer on the form.

Has a doctor told you to avoid anything, or does anything hurt? If "my knee is a little off": When does it hurt, and has anyone looked at it?

Who at home is on board, and what would make you quit? I write the answer down.

How ready are you to start this week, 1 to 10? If 6: Why not a 3?

Decide what each answer changes

Every answer should change the program, or it wasn't worth asking. Some answers mean you stop and get a doctor's okay before you train them.

How to do it

  1. List the answers you hear most and write what each one changes in the program.
  2. Write one sentence for each that tells the client what changed and why. That's how they know you listened.
  3. Learn the red flags below. If you hear one, get a doctor's okay in writing before you train them.
  4. Check the kinds of clients you train and add their extra lines to your form.

Example: Maya's answer rules

Desk job: more work for hips and upper back. I say: "You sit all day, so we'll start with your hips and upper back."

Sleeps 6 hours: fewer hard sessions and more walking at first.

Quit twice around week 3: a check-in planned for week 3.

Says blood pressure isn't under control: doctor's okay in writing first.

Online client: lists equipment and sends a short squat video before we start.

My refer-out rule in one sentence

Asking for a doctor's okay

, thanks for telling me about . Before we train, I need a written okay from your doctor. That's not me saying no. It's me making sure I don't hurt you. Once you have it, we start on .

Write your close

My first sale, Miss Gertrude, took about two hours of me talking, giving her the moon. Two hours of you talking is not a consultation, so close with their pain, their finish line, and one clear question.

How to do it

  1. Pull their pain and their finish line from your question sheet, in their words.
  2. Fill in the close below. Sell the vacation, not the flight: no sets and reps.
  3. Say the price once, then stop talking.
  4. If they pause, ask: "What part are you unsure about?"
  5. If money comes up, don't cut your price. Add value instead, like an extra weekly check-in.

Example: Maya closing with Dana

So the real problem is your knee aches and you're wiped out by 3pm at your desk, right?

And if we fixed it, you'd finally go on hikes with your kids without stopping?

Do you want help getting there?

Here's the plan: two sessions a week plus a walking plan. It's $320 a month. Does that work for you?

(Stops talking.)

If money comes up: The price stays the same, and I'll add a weekly check-in call.
